Accident due to uneven pavement between Joules / Waitrose

Sent to Bath and North East Somerset Council less than a minute later. Council ref: 2675254.
My 84 year old mother tripped on the uneven paving slabs shown in these photos causing her terrible bruising to her mouth and chin which required hospital treatment and her chin to be glued. She is also very shaken by the incident. There is a wide gap around this paving slab making it a serious trip hazard which needs rectifying before any further accidents occur.
