Drain cover , near lampost with 136 stamped on it, on durley hill

The drain cover has sunk into pavement causing a trip hazard which happened to me. I don't know how long it's been like it. It caused myself to trip and fall flat out causing extreme injuries to my face as photos show....I have plenty more but you only allow 3 photos
