Cracked/uneven pavement - Cleveland Cottages

Sent to Bath and North East Somerset Council less than a minute later. Council ref: 3534163.
Incident on pavement along Cleveland Cottages; It was outside the Methodist Church beside the road for Cleveland Cottages; his wheel got stuck in a crack after slipping on an uneven piece of pavement edging tipping him clean out of the chair & onto his face. I was driving past, saw it happen so pulled over to help. It took three adults to collect his chair then get him back into it; he must have been at least 14 stone or more and totally unable to move unaided, legs wrapped in bandages. The poor man was really distressed as you can imagine, he said that it 'wasn't the first time' such a thing had happened in the city which is outrageous.
If you could get someone to look at the dropped kerbing along Cleveland Cottages and make it safer then possibly such a thing may not happen again.
