Significant cracking and potholes causing significant health and safety issues outside school

Sent to Bath and North East Somerset Council less than a minute later. Council ref: 4091278.

A new zebra crossing and pavement drop down was installed less than a year ago. As noted from the pictures, it is significantly deteriorating and, as a result, posing a serious health and safety risk to pedestrians and the parents/carers and, moreover, children of our school. Given that the works are less than a year old and expensive, I would expect that the contractors in question to repair at no cost.
