Crumbling concrete drain cover

The top of this cover has worn away so there are no markings visible. I was hoping that this would be fixed / replaced when Truespeed dug up the pavement recently but they seem to have worked around it! The loose concrete chips and the dip that's worn into it make it a double trip hazard.
