Hollow in pavement surface allowing rain to funnel down our driveway

Sent to Bath and North East Somerset Council less than a minute later. Council ref: 5161746.
The pavement surface has subsided which, during rainfall events, allows water to accumulate in a hollow at one side of our driveway entrance and then overflow down our drive and on into our back garden, taking mud and debris with it.
