Damaged manhole cover moving and causing noise

A damaged manhole cover on Bath Road moves visibly whenever a vehicle passes over it. The surrounding road surface looks damaged (can be seen in photo). The movement also makes a loud noise. This cover is positioned in such a way that most vehicles run over it with their right-hand-side tyres, so this is a continual noise at any time of day or night.
