Broken Pavement outside of Ryman's - caused my dad to fall head first into the road and ended up in hospital

Sent to Bath and North East Somerset Council less than a minute later. Council ref: 7685308.
The pavement is seriously broken and a huge trip hazard. my 78-year-old dad fell head first into the road, causing a head, eye, face, hand and knee injury. And had to go to RUH. Many passersby and workers in the area said this pavement has been a serious problem for a while and they have seen other people fall. Please repair immediately.
