Damaged/slippery surface causing lots of falls

Sent to Bath and North East Somerset Council less than a minute later. Council ref: 8376253.
I work opposite the entrance to Beehive Yard and can see the entrance off Walcot St from my place of work. I have seen numerous people slip/fall on the section of tactile paving there over a number of years - i've had to go out and help people up in the past, one older lady just last week. It's angled down (from pavement level to road level) and there seems to be a specific slab that people slip on. It's on the left hand corner of the entrance. I'm not sure if it's actually damaged/faulty but it definitely needs looking at - i'm happy to show someone exactly where the issue is
