Manhole cover very unstable and edging tarmac broken away

Sent to Bath and North East Somerset Council less than a minute later. Council ref: 9031858.

This manhole is run over every 10 seconds by a car and makes a loud CLANK - it wobbles and is unsafe. Inspection shows tarmac at sides of manhole are cracked open, so it could collapse. This is urgent - it's been like this for weeks and been reported several times already.
